Medicare & Medicaid Information

We are able to bill Medicare and Medicaid when medically necessary services meet applicable coverage requirements.

Coverage is determined by:

• Your diagnosis

• Medical necessity

• Individual insurance plan

• Applicable Medicare or Medicaid guidelines

Coverage cannot be guaranteed until eligibility and benefits have been verified.

Services That May Be Covered

Depending on your diagnosis and insurance plan, covered services may include treatment related to:

• Diabetes with qualifying foot conditions

• Peripheral neuropathy

• Peripheral arterial disease

• Foot ulcers

• Wounds

• Other medically necessary foot conditions

Services Generally Not Covered

Routine maintenance foot care performed solely for personal hygiene or comfort may not be covered by Medicare or Medicaid.

Examples may include:

• Routine nail trimming

• Cosmetic nail care

• Routine callus reduction

• Preventive foot care without a qualifying medical diagnosis

Patients are responsible for any non-covered services.

Advance Beneficiary Notice (ABN)

​

If Medicare is expected not to cover a service, an Advance Beneficiary Notice (ABN) will be reviewed with you before treatment.

Signing an ABN allows you to choose whether to receive the service while acknowledging potential financial responsibility if Medicare denies payment.

ABNs are provided only when required by Medicare guidelines.
