Initial Wellness & Health Optimization Appointment

$299 ($199 with same day  program enrollment)

 

This comprehensive 90-minute appointment is designed to establish care and create a personalized plan to help you optimize your health, prevent disease, and improve your overall well-being. During your visit, your board-certified nurse practitioner will review your medical history, current medications, lifestyle, nutrition, sleep, stress, laboratory results (when available), and personal health goals to develop an individualized healthcare plan.

Your appointment may include nutrition and lifestyle counseling, exercise recommendations, laboratory ordering, body composition and metabolic assessment, hormone and nutritional evaluation, supplement recommendations, peptide therapy evaluation (when clinically appropriate), and other evidence-based health optimization strategies to help you feel and function at your best.

Following your appointment, you may choose the care option that best fits your needs.

Option 1: 12-Month Wellness & Health Optimization Program (Best Value)

Patients who enroll in the 12-Month Wellness & Health Optimization Program during their initial appointment receive a $100 credit toward their initial appointment.

The program is $100 per month with a 12-month commitment and includes:

• Unlimited Wellness & Health Optimization follow-up appointments

• Initial, routine, and clinically indicated laboratory testing (not including any  specialty testing, and labs outside of fullscript/quest formulary)

• Personalized healthcare planning and treatment optimization

• Structured follow-up schedule for best results

• Unlimited secure EMR messaging with your healthcare provider

• Nutrition, lifestyle, sleep, stress, and wellness coaching

• Supplement recommendations and discounts on professional-grade supplements

• Ongoing provider support and individualized treatment recommendations

• Access to functional and integrative testing, treatments and healthcare planning.

Peptides, supplements, hormones, and other therapies are not included in the program fee. If prescribed or recommended, these products are billed separately based on the specific treatment selected and current pricing.

Program enrollment is available through:

• $100 monthly EFT/ACH payments for 12 months, or

• One annual payment with a 5% paid-in-full discount.

Option 2: À La Carte Care

Patients who prefer not to enroll in the program may continue with à la carte follow-up appointments, allowing you to receive care only when desired.

À la carte follow-up appointments are billed individually and do not include the benefits of the Wellness & Health Optimization Program, such as unlimited visits, any included laboratory testing, unlimited EMR messaging, supplement discounts, or ongoing structured support. Recommended therapies, supplements, peptides, hormones, and laboratory testing are billed separately when applicable.

After successfully completing the 12-Month Wellness & Health Optimization Program, patients may choose to transition into a Wellness & Health Optimization Membership for continued maintenance and long-term support or continue with à la carte follow-up appointments.

Optional Therapies

Peptides, hormone optimization, supplements, specialty laboratory testing, and other recommended therapies are available when clinically appropriate. These services and products are not included in the program fee and are billed separately based on the specific treatment or testing selected.

Program Pricing

• Adults (18 years and older): $100 per month

• Children and Adolescents (Under 18 years): $75 per month

All programs require a 12-month commitment and are paid through automatic EFT/ACH payments.

Patients may also choose to pay the entire 12-month program in advance and receive a 5% paid-in-full discount.

Patients enrolling during their Initial Wellness & Health Optimization Appointment receive a $100 credit toward their initial appointment.

Upon successful completion of the 12-month program, you may choose to transition into a Wellness & Health Optimization Membership for continued maintenance and ongoing support or continue care through à la carte follow-up appointments as your healthcare needs evolve.

Our À La Carte Wellness & Health Optimization Follow-Up Appointments are ideal for patients who prefer the flexibility of paying only for the care they want without enrolling in a structured program or membership.

Each follow-up appointment includes a personalized assessment of your progress, health goals, lifestyle, nutrition, sleep, stress, current therapies, and treatment response. Your provider will continue to optimize your personalized healthcare plan, review laboratory results when appropriate, and provide individualized recommendations to support your long-term health and wellness goals.

À La Carte Includes

• Individual Wellness & Health Optimization follow-up appointments

• Personalized healthcare planning and treatment recommendations

• Hormone, peptide, supplement, and wellness therapy management as clinically appropriate

• Nutrition and lifestyle counseling
