Direct Care Initial Visit

Ideal for patients seeking personalized, relationship-based healthcare outside the traditional insurance model. During your initial visit, you'll receive a comprehensive evaluation and work with your provider to develop a care plan tailored to your health goals.

You may choose to:

• Pay the standard visit fee and continue with care on an as-needed, à la carte basis, or

• Enroll in our 12-month Direct Care Membership during your visit and receive 50% off your initial appointment, along with the ongoing benefits of membership.

Our Direct Care model offers transparent pricing, longer appointments, and convenient access to your healthcare provider without the restrictions of traditional insurance.

• Unlimited office visits for acute care ( sick, injury, rash) chronic disease management (diabetes, blood pressure, depression), preventive care (pap, annual physical/labs), and wellness (health goals, health education, advance care planning)

• No insurance billing, copays, deductibles, or prior authorizations

• Longer, personalized appointments with your board-certified nurse practitioner

• Routine acute and preventive laboratory testing included when appropriate

• Additional discounted laboratory testing through Fullscript and Quest Diagnostics

• Unlimited secure messaging through the patient portal (EMR)

• Unlimited follow-up questions and ongoing support

• Faster appointment availability and flexible scheduling

• Continuity of care with a provider who knows you and your health goals

Membership Pricing

• Initial Visit: $200

• Monthly Membership: $100 per month

• Membership Term: 12-month commitment

• Payments are processed by electronic bank transfer/debit (EFT/ACH) only.​

As a Direct Care Member, you'll never pay another specialty initial consultation fee. Once you're established with our practice, consultations for Weight Management, BHRT, and Health & Wellness Optimization Programs and Memberships can be completed during your included Direct Care visits. Simply enroll in the program or membership of your choice at the discounted Direct Care Member rate—saving you both time and money while keeping all of your care coordinated through one trusted provider.
