This comprehensive 90-minute appointment is designed to establish care and create a personalized plan to help you achieve safe, sustainable weight loss and improved overall health. During your visit, your board-certified nurse practitioner will review your medical history, current medications, lifestyle, previous weight-loss efforts, laboratory results (when available), and health goals to develop an individualized treatment plan.

Your appointment may include nutrition and lifestyle counseling, exercise recommendations, laboratory ordering, body composition and metabolic assessment, evaluation for GLP-1 peptide therapy (when clinically appropriate), supplement recommendations, and other health optimization strategies to support long-term success.

Following your appointment, you may choose the care option that best fits your needs:

Option 1: 12-Month Weight Loss & Management Program (Best Value)

Patients who enroll in the 12-Month Weight Loss & Management Program at their initial appointment receive a $100 credit toward their initial appointment.

The program is $100 per month with a 12-month commitment and includes:

• Up to 1 monthly Weight Loss & Management follow-up appointments

• Initial, routine, and clinically indicated laboratory testing

• Structured follow-up schedule for accountability, progress monitoring, and treatment optimization

• Unlimited secure EMR messaging with your healthcare provider

• Personalized medication management and dose adjustments

• Nutrition and lifestyle coaching

• Discounts on professional-grade supplements

• Ongoing provider support and individualized treatment recommendations

GLP-1 peptide medications are not included in the program fee. If prescribed, medications are billed separately, with pricing based on the medication selected, dosage, and current pharmacy pricing.

Program enrollment is available through:

• $100 monthly EFT/ACH payments for 12 months, or

• One annual payment with a 5% paid-in-full discount.

Option 2: À La Carte Care

Patients who prefer not to enroll in the program may continue with à la carte follow-up appointments, allowing you to receive care only when needed.

À la carte follow-up appointments are billed individually and do not include the benefits of the Weight Loss & Management Program, such as unlimited visits, included laboratory testing, unlimited EMR messaging, supplement discounts, or ongoing structured support. If prescribed, GLP-1 peptide medications are also billed separately.

After successfully completing the 12-Month Weight Loss & Management Program, patients may choose to transition into a Weight Loss & Management Membership for continued maintenance and long-term support or transition into an ala carte follow up plan option.

Compound GLP-1 Peptide Medications

When prescribed and clinically appropriate, will be ordered and delivered by partner pharmacy. There are not included in the price of the monthly care service fees. GLP-1 peptide medications are available as an optional part of your treatment plan. Medication costs are separate from the program fee and vary depending on the medication prescribed, dosage, and current pharmacy pricing.

Your provider will make medication adjustments as clinically appropriate and continue to support your health optimization goals.
